Streamlined add-on setup flow

We've made much more easier for admins to finish Smart Checklist add-on setup right from issue view with two-steps wizard.

Step 1

Script automatically verifies whether "Checklists" custom field created and let admin create it by one button click.

Step 2

Then Admin can choose proper screens to associate recently created "Checklists" custom field with.

And then success!
